If you are completing your Certificate at the end of the Spring 2023 semester this is the deadline to email nutritionregistrar@tufts.edu your name as it should be printed on your official Certificate and your Certificate mailing address. You will then be emailed a Qualtrics link to complete the Declaration of Completion online form, as well as the Exit Evaluation online form.
