If you are completing your Certificate at the end of the Fall 2022 semester (which will have a February 2023 conferral date) this is the deadline to email nutritionregistrar@tufts.edu your name as it should be printed on your official Certificate and your Certificate mailing address. You will then be emailed a Qualtrics link to complete the Declaration of Completion online form, as well as the Exit Evaluation online form.
